In honor of #GivingTuesday, The Florida Blue Foundation is giving you the chance to nominate your favorite charity to be one of five selected to receive $10,000 by hosting their own #FloridaGives campaign.

Here's how you can enter Unbroken Horizons Scholarship Foundation and share why you are grateful that our nonprofit exists...

1) All week, December 1 through December 6, log on to spr.ly/FloridaGives

2) Enter Unbroken Horizons Scholarship Foundation and the reason you are grateful for us

3) Submit throughout the week to give us the best chance possible of winning

4) Share your story via social media, using the hashtag #FloridaGives to help spread the word

The charity winners will be announced on December 10.
